The World Health Organization has updated its guidelines on dementia risk reduction, adding seven new risk factors such as stroke, sleep apnea, HIV, and air pollution. This update reflects the latest evidence in cognitive health and emphasizes the importance of various health conditions and lifestyle factors in managing dementia risk.

The World Health Organization (WHO) has released significant updates to its guidelines aimed at reducing the risk of cognitive decline and dementia. This second edition of the original 2019 report incorporates new evidence and advancements in understanding dementia risk factors. The updated guidelines now include seven additional health conditions and exposures that may contribute to dementia risk, alongside the previously identified twelve factors.

Among the newly recognized risk factors are stroke, sleep apnea, untreated HIV, and exposure to air pollution. These additions underscore the multifaceted nature of dementia risk, which encompasses not only neurological health but also cardiovascular and environmental influences. Dr. Daniel Amen, a psychiatrist and founder of Amen Clinics, discussed these updates, emphasizing the importance of maintaining cardiovascular health and addressing sleep issues as part of a comprehensive approach to cognitive health.

The WHO’s guidelines highlight that conditions affecting blood flow, such as heart disease and stroke, can significantly impact brain health. Dr. Amen pointed out that any blockage in blood flow can increase the risk of dementia. He also noted that erectile dysfunction, which affects a significant percentage of older adults, may indicate underlying vascular issues that could also relate to cognitive decline.

Additionally, the WHO does not recommend menopausal hormone therapy specifically for dementia prevention in women aged 65 and older, citing insufficient evidence. However, Dr. Amen acknowledged that hormonal changes during menopause could affect mood and memory, although the direct relationship between hormone therapy and dementia risk remains unclear.

The updated guidelines also stress the role of sleep in brain health. Sleep disorders, particularly sleep apnea, can lead to changes in the brain that mimic Alzheimer’s disease. Research suggests that adequate sleep may facilitate the clearance of beta-amyloid, a protein associated with Alzheimer's, although the exact relationship between sleep and dementia continues to be an area of active study.

Moreover, the WHO has recognized the impact of environmental toxins, including air pollution, on cognitive health. Long-term exposure to pollutants has been linked to an increased risk of cognitive decline, although the evidence is characterized as low to very low certainty.

Dr. Amen emphasized that protecting the brain from trauma is crucial, as even mild traumatic brain injuries can have lasting effects on mental health and cognitive function. He advocates for lifelong engagement in activities that promote brain health, such as social interaction and continuous learning.

As dementia risk escalates with age, with estimates suggesting that one in five individuals may develop dementia by age 85, the WHO’s updated guidelines serve as a vital resource for understanding and mitigating these risks. Dr. Amen encourages individuals to consider their lifestyle choices and their potential impact on brain health, advocating for proactive measures throughout life to promote cognitive well-being.
